Alternatively, you might be able to use speech to text software, then manually tidy it up. For some reason, medical transcription is considered a separate problem with specialised tools.

You use the foot pedals to play, pause, reverse, fast forward, fast reverse. Ideally when you go fast forward, the software adjusts to avoid a frequency change in the voice.

The software may include a small text editor, but it need no do. You could use any text editor or word processor.

The profusion of sound and video formats reminds me of something Woody Allen might have said: In the kamasutra, I noticed that positions 149 and 211 are the same, except the woman has her fingers crossed.. It is wicked to create a new data format unless it is significantly better than what already exists.
